Medicare in San Luis Obispo County, in numbers
About 74,000 people in San Luis Obispo County were on Medicare in June 2026, according to the CMS Medicare Monthly Enrollment data. About 24 percent of them were in a Medicare Advantage plan, against about 51 percent across California.
That share has grown faster here than in either neighboring county. It was under 10 percent in 2013 and has more than doubled since. Even so, roughly three out of four people on Medicare in this county are on Original Medicare, most of them with a Medicare Supplement and a standalone Part D drug plan.
It is also one of the older counties in the state. More than one resident in five is 65 or older, according to the Census Bureau, against about one in six statewide. We help people here with Medicare Supplement, Medicare Advantage and Part D, and most of our Medicare clients choose a Supplement.
Supplement or Advantage in San Luis Obispo County
The deciding question is the same everywhere: are the doctors and hospitals you use in the plan's network, this year, in this county? What makes this county different is the shape of the Advantage market. For 2027, every standard Medicare Advantage plan on the CMS plan list for San Luis Obispo County is an HMO. There is no Advantage PPO here.
An HMO generally means a primary care doctor who coordinates your care, referrals to see specialists, and no coverage outside the network except for emergencies and urgent care away from home. That suits some people well. It suits people less well who see a specialist in Santa Barbara, Los Angeles or the Bay Area, or who spend part of the year away.
A Medicare Supplement has no network at all. Any doctor or hospital in the country that accepts Medicare accepts it, and no referrals are needed. Plan G and Plan N are the two we are asked about most, and Plan G vs Plan N compares them.
What changes here for 2027
CMS published its list of 2027 plans in September. It shows one national insurer withdrawing its Medicare Advantage plans from San Luis Obispo County, and from Santa Barbara and Ventura counties as well. CMS says the list can still change while contracts are finalized.
If your Advantage plan is leaving, it will send you a letter this fall. You will have a special enrollment period to choose another plan, and when a plan leaves the area you also gain a time limited right to buy certain Medicare Supplement policies without answering health questions. That right is short, so call us as soon as the letter arrives rather than waiting for the Annual Enrollment Period to end.
The Annual Enrollment Period runs October 15 to December 7. Changes made then take effect January 1.
Doctors and hospitals to check before you choose
Two hospital systems run the county's four general hospitals. Write down the names you intend to keep, including the hospital, and we will check them with you.

Dignity Health. French Hospital Medical Center in San Luis Obispo and Arroyo Grande Community Hospital, with Dignity's Pacific Central Coast Health Centers clinics.

Adventist Health. Adventist Health Sierra Vista in San Luis Obispo and Adventist Health Twin Cities in Templeton. Both were Tenet hospitals until Adventist Health bought them in 2024, so older paperwork may show the old names.

Physician groups. Coastal Communities Physician Network, part of Heritage Provider Network, and Adventist Health's own clinics. From January 1 2026 Adventist Health's clinics stopped contracting with Heritage, which affected about 6,500 patients, according to KSBY. It is a good example of why a network has to be checked every year.

Kaiser Permanente does not operate in San Luis Obispo County and offers no Medicare plan here. If you are moving from a Kaiser area, your Kaiser Medicare plan will not follow you, and the move opens a special enrollment period to choose something new.
