- What to expect:
- After you select the plan you want, you will need to set up a username and password.
- You will need names, dates of birth and social security numbers for all family members on the health plan.
- At the end of the application, you will pay your initial premium with checking account information, Mastercard or Visa.
- Note: After you have received your new insurance card, we recommend you call Blue Shield to set up Auto-Pay. Use the phone number on the back of your new insurance card.
- IMPORTANT NOTE: When you apply with a NEW insurance company for a plan that begins on 1-1-2020, be sure to cancel your old coverage during the month of December.
